2017 Grant Report
Unless otherwise indicated, organizations receiving grants in the amount of $2,500 or more must submit a grant report detailing activities supported by the Foundation during the grant year. Reporting requirements and deadlines are included in your award letter and are outlined below. Failure to submit this report on time will affect your organization's eligibility for future funding.
CHANGES TO GRANT REPORT PROCESS
In an ongoing effort to streamline operations and simplify reporting requirements, please note the following changes to the grant report process:
Effective 2016, scholarship grant recipients need NOT submit a Siragusa Family Foundation grant report form as required in prior years. Instead, the Foundation requests a short (1-2 paragraphs) update at the end of the school year with information about the scholarship recipient(s) and any changes to your scholarship program. Updates may be sent via email to firstname.lastname@example.org and are expected by September 1, 2017.
New Special Initiative Grants
Effective 2016, special initiatives need NOT submit a Siragusa Family Foundation grant report form. Instead, the Foundation requests periodic updates throughout the year and may require a proposal to be considered for future funding.
