2011 Year-End Grant Report
The foundation requires its grantees to complete a year-end grant report to detail activities supported by the foundation in the past year.
Please note that the 2011 Year-End Grant Report is due by March 1, 2012. Failure to submit this report on time will affect your organization's future funding.
Please follow these instructions:
- Save the word document below as new file and title the file "[Your Organization's Name] GR 2011" prior to entering information.
- When you have completed all ten (10) questions within the report, send as a Word document via email to info@siragusa.org with the subject same as the file name no later than Thursday, March 1, 2012.
- Please complete all ten (10) questions in this report.
