The foundation requires its grantees to complete a year-end grant report to detail activities supported by the foundation in the past year.

 

Please save the word document below as new and title the file with your "Organization Name GR 2008" prior to entering information.

 

Upon completion, send as a Word document via email with the subject same as the file name to info@siragusa.org no later than Friday, January 16, 2009.
